§ 150.233 REGISTRATION FORMS.
   (A)    Applications for registration shall be made in such form and in accordance with such instructions as may be provided by the Building Department and shall include at least the following information:
      (1)   The name, address, telephone number, date of birth and copy of government issued identification of owner and resident agent (if applicable);
      (2)   The names, addresses and telephone numbers of all owners of the rental dwelling;
      (3)   The name, address and telephone number of the responsible resident agent; and
      (4)   The number of rental units in each rental dwelling.
   (B)   No post office box will be accepted as a legal address. The registration shall be kept in the Building Department with a copy provided to Code Enforcement. Applicants will also be informed of certificate of compliance requirements at the time of registration.
