§ 150.215  APPLICATION.
   A fully and accurately completed application for a permit under this section, in form approved by the Department of Building and Structural Safety, shall be made to the Department of Public Services and shall be accompanied by an inspection fee as established by resolution of the City Council. The application shall be signed by the contractor proposed to do the moving, by the owner of the structure and by the applicant (if other than the contractor proposed to do the moving or the owner of the structure) and shall contain at least the following information:
   (A)   The name and address of the owner of the building or structure to be moved.
   (B)   The name and address of the contractor proposed to do the moving.
   (C)   A site of origin plan which shall designate the present location and description of the building site or structure to be moved and all other buildings or structures as on the site of origin, including, but not limited to, swimming pools, sheds, outbuildings, underground storage tanks, including the name and address of the owner of the property on which the building is located if it is within the city.
   (D)   A description of the lot or parcel to which the building or structure will be moved, including the name and address of the owner of the lot or parcel if it is within the city.
   (E)   A certified copy of any building or use restrictions applicable to the site of destination.
   (F)   A statement as to whether or not the contractor proposed to do the moving has ever previously acknowledged, admitted or been judicially determined to have violated one or more provisions of this subchapter or similar provisions of another governmental unit.
