§ 95.01 PERMIT REQUIRED; FEE.
   (A)   It shall be unlawful for any person, firm, or corporation to make any excavation on any portions of a right-of-way, street, alley, or private property within the city for the purpose of installing a utility meter or service line or any other type of excavation, without first having obtained from the Building Inspector a permit for the same. The permit issued by the Building Inspector shall designate the exact location of the proposed installation and shall be issued after an actual physical survey by the Building Inspector has been made.
   (B)   Any unusual condition of the street or other right-of-way shall be noted on the permit so that after the construction is completed and final inspection is made by the Building Inspector as to restoration, a record of prior condition shall have been established.
   (C)   The fee to be charged by the city for such excavations shall be payable to the City Treasurer for administration of this section.
