The City Manager may issue a license to any person, firm or corporation within the time prescribed by this chapter, after this application has been filed, to conduct or operate a public dance hall, billiard and pool room or bowling alley. The application shall contain the full name and address of all parties financially interested, also the name or names of the active manager or person in charge with their addresses. The application shall also state the following:
(A) The premises where said dance hall, billiard and pool room or bowling alley is to be conducted.
(B) Whether the applicant has ever been engaged in operating a dance hall, billiard or pool room or bowling alley, and when, where and how long in each place within the last five years.
(C) Whether the applicant or any one financially interested or designated to act as manager of the dance hall, billiard and pool room or bowling alley has during the five years then last past been convicted of a violation of the law of this state or this code and any other ordinance of the city, and separate statement of each conviction, nature and offense and punishment given.
(D) The name of the owner of the premises on which the dance hall, billiard and pool room or bowling alley is located. The application shall be signed by the applicant or applicants or if a firm or corporation, by the manager and any of its officials.
