An application for a dance license shall be filed with the city on forms as are specified by the City Administrator and shall specify the names and addresses of the person, persons, committee or organization that is to hold the dance, the time and place thereof, the area of the dance floor, the estimated number of dance patrons and other information as the city may reasonably request.
