§ 78.50 REQUEST FORM REQUIRED.
   (A)   Any individual who desires to rename a city street shall submit a request form to the Board of Public Works and Safety ("Board").
   (B)   The request form must contain, but need not be limited to, the following:
      (1)   Individual's name, address and telephone number;
      (2)   The specific location of the street to be renamed;
      (3)   The proposed name for the street; and
      (4)   Any historical background or other information supporting the proposed name.
   (C)   The request form shall be in the form adopted by the Board.
