Each applicant for a taxicab operator's license shall present and file with the Manager his signed application setting forth:
(a) The trade name under which he intends to operate;
(b) The number of vehicles he intends to operate;
(c) A general description of each vehicle for which the license is desired;
(d) Markings and/or lettering inscribed on each vehicle to be licensed; and
(e) Any additional information deemed by the Manager as pertinent to the issuance of such license.
