§ 127.052 APPLICATION PROCEDURE.
   (A)   Application for the permit required by § 127.051 shall be made in writing to the City Clerk; however, no application shall be accepted from any person who is less than 16 years old.
   (B)   The application shall be accompanied by the following:
      (1)   Medical certificate issued not more than ten days prior to the date of the filing of the application, indicating that the applicant is in good health and free from any communicable disease.
      (2)   Hollywood police identification civilian registration number.
      (3)   Evidence, by diploma, certificate, or affidavit, that the applicant has received at least 400 hours of practical training and experience in the practice of electrolysis, extending over a period of not less than six months, either in a school in which the subject is taught, or under the personal supervision of a licensed electrologist who has practiced not less than five years.
      (4)   Evidence that the applicant has complied with the liability insurance requirement of this subchapter.
