5-1-2: VITAL STATISTICS RECORDS:
   A.   Registrar Of Vital Statistics: The village clerk shall be the local registrar for the purpose of recording vital statistics, and he shall be governed by 410 Illinois Compiled Statutes, public health. Said local registrar shall cooperate with the Cook County department of public health and shall be responsible for the issuance of permits for the burial or removal of bodies.
   B.   Deputy Registrar: The local registrar shall appoint a deputy to be known as the deputy local registrar who shall act in case of the local registrar's absence, illness or disability.
   C.   Birth Reports: It shall be the duty of every physician or midwife who attends the birth of a child, to report the same to the local registrar within thirty (30) days from the date of such birth. Such reports shall be made in writing on blank forms to be furnished by the registrar and shall give the name and date of birth of such child and such other information as the registrar shall require.
