5-1-8: REPORTING ACCIDENTS, DISTURBANCES AND REQUESTS FOR AID:
Whenever any accident, disturbance, or request for aid shall occur within the village, police officers at the scene shall make a written report to the police chief, including all relevant information and as much of the following detail as may be applicable:
   A.   The place, day, time, and nature of the incident;
   B.   Weather conditions and lighting at the scene of any accident, including whether streetlights were lit;
   C.   The name and residence of any person injured and the nature and extent of the injury suffered;
   D.   The names and residences of principal witnesses; and
   E.   The name and residence of any attending physician. (Ord., 12-16-1991)
