8-8-4: APPLICATION FOR PERMIT; FEES:
   A.   Forms: The city clerk shall provide application forms to request permits hereunder. Such application forms shall include all reasonable information needed by each and every city department to evaluate all matters within its area of responsibility.
   B.   Time Of Filing: The application form shall be submitted to the city clerk at the city hall no later than sixty (60) days prior to the commencement of the special event; except, that large scale events that require extensive planning shall be submitted no less than six (6) months prior to the commencement of the special event. Applications for small scale special events which are not filed within the sixty (60) day deadline may be provisionally accepted, subject to refusal if any city department does not have sufficient time to process the application. Any application for a special event which, because of its size or scope, requires more extensive planning than normal, may be rejected if an insufficient time for advance planning is available for any city department. No application shall be submitted more than thirteen (13) months in advance. (Ord. 95-O-11, 5-2-1995)
   C.   Fees: Each application for a permit for a special event shall include full payment of a filing fee as provided in section 3-2A-1 of this code for each day on which the special event will take place or continue. (Ord. 95-O-11, 5-2-1995; amd. 2009 Code)
   D.   Return Of Application: An application shall be considered incomplete and may be returned without any action by the city if:
      1.   The application is not fully completed or any requested information is missing or incomplete; or
      2.   Full payment of the required fee is not tendered with the application; or
      3.   The insurance documents required under this chapter are not complete and attached. (Ord. 95-O-11, 5-2-1995)
