4-19-6: APPLICATION:
   A.   Applications for all licenses and permits required by ordinance shall be made in writing to the city manager in the absence of provision to the contrary. Each application shall state the name of the applicant (and in the case of a partnership, the name and addresses of all partners and in the case of a corporation, its president, secretary, and registered agent), the type or nature of the business, the location or premises to be used, if any, federal income tax number, Illinois business tax number, after hours emergency contact information, the time covered and the fees to be paid. Each application shall contain any additional information as may be needed for the proper guidance of the city officials in issuing the requested license or fulfilling the purposes of this chapter.
   B.   It shall be unlawful for any person to knowingly give false information or falsely affirm or swear to any matter in the application for a license or obtain a license by fraudulent means, or to display a fictitious or fraudulently altered license or permit, or otherwise to misuse any license. The corporate authorities shall, pursuant to the provisions of this chapter, suspend or revoke the license of any person, firm, or corporation who violates the provisions of this section and any suspension or revocation shall be in addition to any and all other penalties hereinafter provided.
   C.   If at any time during the term of any license the information required by this section changes in any significant way, such as, but not limited to, a change of address or the conviction of an applicant of a felony, then the applicant, within five (5) days after such change of information, shall provide the updated or revised information to the city manager.
