4-4-5: APPLICATION FOR LICENSE; FEE:
   A.   Forms for requesting a raffle license hereunder shall be made available at city hall and shall be filed by any organization seeking a license hereunder.
   B.   The application shall contain a sworn statement attesting to the not for profit character of the applicant organization, which shall be signed by the presiding officer and secretary of that organization.
   C.   The application shall specify, among other things:
      1.   All persons who are in any position of authority within the applicant organization, such as, but not limited to, director, officer, employee, or agent;
      2.   All persons who shall participate in the management of the raffle in any way;
      3.   The single raffles manager required by subsection 4-4-9G of this chapter;
      4.   The person who shall account for gross receipts, expenses and net proceeds from each raffle pursuant to subsection 4-4-9H5 of this chapter;
      5.   The aggregate retail value of all prizes or merchandise to be awarded by the licensee in a single raffle;
      6.   The maximum retail value of each prize to be awarded by the licensee in a single raffle;
      7.   The maximum price which may be charged for each raffle chance to be issued or sold;
      8.   The maximum number of days during which chances may be issued or sold;
      9.   The area or areas within the city in which raffle chances will be sold or issued;
      10.   The time period during which raffle chances will be sold or issued;
      11.   The time of determination of winning chances and the location or locations at which winning chances will be determined; and
      12.   The method of determining winning chance(s). (Ord. 2002-O-36, 5-21-2002)
   D.   The fee for the initial filing of an application shall be as provided in section 3-2A-1 of this code, which may, however, upon good cause shown, be waived by the mayor. (Ord. 2002-O-36, 5-21-2002; amd. 2009 Code)
