1331.03 APPLICATION FOR PERMIT; PLANS AND PERMIT FEE.
   (a)    The owner, part owner, or occupant of any lot, premises or parcel of land within the Municipality who desires to construct or erect a satellite dish antenna at any location within the City or any contractor hired by such owner, part owner or occupant to construct or erect a satellite dish antenna, shall apply to the Building Commissioner of the City for the permit referred to in Section 1331.02.
   (b)    The Building Commissioner shall issue such permit provided the applicant:
      (1)    Submits a written application with a plot plan of the lot, premises or parcel attached showing the exact location of the proposed satellite dish antenna; a description of the kind of satellite dish antenna, and the plans and specifications showing the elevations, where it is to be erected, and of the dish itself, and sufficient details to show the method of assembly and construction. Each set of plans and specifications shall give the address where the satellite dish antenna is to be installed, name and address of the owner of the premises where the satellite dish antenna is to be installed, and of the person who prepared the plans and specifications. Each application shall indicate the owner of the premises, the occupant of the premises and the contractor or other person to be permitted to construct or erect the proposed satellite dish antenna, who shall be a licensed contractor, pursuant to the regulations of the City.
      (2)    Meets all of the requirements of this chapter.
      (3)    Submits a check in the amount of sixty dollars ($60.00) along with each application which represents the permit fee. The permit fee shall cover the cost of review of the plans by the Architectural Board of Review and the handling of the application and inspection of the construction by the Building Commissioner.
