913.02 APPLICATION INFORMATION.

   Application for a permit to make any improvement listed in Section 913.01 shall be made in writing and contain the following information:

   (a)    Names and addresses of all owners of land involved;

   (b)    Name and address of the applicant;

   (c)    Names and addresses of persons performing the work, and

   (d)    A statement of the reason for making the improvement specified.
