1329.02 APPLICATION; INFORMATION.
   (a)   Every applicant for registration to engage in the trades referenced in Section 1329.01 through contracting and/or hire, shall make application at the office of the Building Chief on forms prescribed by the Building Chief, and upon the approval of his or her application furnish and file with the Building Chief a bond in the amount of twenty-five thousand dollars ($25,000) on a form as set forth in Section 1329.18 and as set forth in the fee structure in Chapter 105, Section 105.08 of the Administrative Code for each category of registration. Such bond shall be approved as to form by the Director of Law and as to sufficiency of surety by the Mayor, guaranteeing full and faithful compliance by the applicant only to the extent of the bond value with all the provisions and regulations of applicable building codes of the Village, the State of Ohio or any subdivision thereof now or hereafter having power and authority to regulate the construction of buildings or other structures within the Village.
   (b)   Each application for a license shall contain the name of the person applying for registration, the location of his/her place of business, whether such applicant is a member of the firm, partnership, corporation, association or any combination thereof, evidence showing that such applicant is duly authorized to act for such firm, partnership, corporation, association or any combination thereof, proof of insurance as written in the form of a verified copy of the certificate of insurance, a copy of Ohio Worker’s Compensation Certificate, as well as such other information as the Building Chief shall prescribe. Every applicant for registration shall deposit with the Building Department evidence of a public liability insurance policy in the amount of one million dollars ($1,000,000) insuring the Village or client against any loss or damage to persons or property from or growing out of the work being performed. The amount of such insurance shall be as set forth in Section 105.08 of the Administrative Code (The General Fee Schedule). All policies shall be subject to the approval of the Director of Law.
