Prior to the issuance of the permit referred to in Section 735.01, the applicant shall identify its name and legal address. If the entity is a corporation, the corporation charter number, state of incorporation and the names and addresses of all officers shall be on the permit; if a partnership, the date and place where the partnership was registered together with the names and addresses of all partners, whether general or limited, shall be on the permit. The applicant shall pay an annual registration fee as provided in Chapter 105. Each permit holder shall provide a telephone number where a responsible representative of the applicant can be contacted between the hours of 9:00 a.m. to 5:00 p.m., seven days a week.
