941.08 WHERE PROTECTION IS REQUIRED.
   (a)    An approved backflow prevention assembly or method shall be installed on each service line to a consumer's water system serving properties, where in the judgment of the Utilities Director, actual or potential hazard to the public water supply system exists.
   (b)    An approved backflow prevention assembly or method shall be installed on each service line to a consumer's water system serving properties, where any of the following conditions exist:
      (1)    Properties having or having access to second source water.
       (2)    Properties where any substance is handled which can create an actual or potential hazard to the public potable water supply system. This shall include properties having sources or systems containing process fluids or water originating from the public potable water supply system which are no longer under the quality control of the Utilities Director, Division of Water.
      (3)    Properties having internal cross-connections that, in the judgment of the Utilities Director, are not correctable or where intricate or complex plumbing arrangements make it impractical to determine whether or not cross connections exist.
      (4)    Properties where, because of security requirements or other prohibitions or restrictions, it is impossible or impractical to make a complete cross connection survey.
       (5)    Properties supplied by more than one service where the services are connected to the water distribution system within different pressure districts.
      (6)    Properties having a repeated history of cross-connections being established or re-established.
   (c)    If the complete interruption of water through a given service is critical to the customer's operations, an installation of backflow prevention assemblies in parallel is required.
