941.07 TESTING AND RECORDING.
   (a)    Each assembly shall be tested at least annually or more frequently if required by the Utilities Director.
   (b)    Testing shall be performed by a contractor who is certified by the Operator Training Committee of Ohio, Inc. The actual tester must possess an active Ohio Department of Health Backflow Prevention Assembly Testers Certificate and be approved by the Utilities Director.
   (c)    Test and repair records are to be filed by the owner, lessee or official custodian and a copy sent to the City of Heath, Water Distribution Office.
   (d)    Each assembly shall have a tag attached listing the date of most recent test, tester's name and certification number, company tester is employed with, type and date of repairs and test results.
   (e)    A maintenance and test log shall be maintained and include:
       (1)    Date of each test;
      (2)    Name and certification number of the tester;
      (3)    Name of company tester is employed with;
      (4)    Test results;
      (5)    Repairs or servicing required;
      (6)    Repairs and date completed; and
      (7)    Servicing performed and date completed.
   (f)   Test equipment used for initial, annual or emergency Backflow Prevention Assembly testing required by this Rule and Regulation, shall be calibrated at least annually by an independent calibration company approved by the City of Heath, Division of Water.
