GENERAL PROVISIONS
§ 95.01 STREET PLAN ESTABLISHED.
   (A)   There is established an official system of street names in the city as shown on the map entitled Hazard Street Plan, dated July 1, 197 , as produced by the Planning Commission. The street plan is made a part of this chapter as fully as if set out herein. A copy of the street plan shall be maintained in the office of the City Manager or other authorized city official and shall be available for inspection during normal office hours.
   (B)   Names of streets in the city shall remain as shown on the street plan unless officially changed by specific ordinance.
   (C)   No new streets shall be accepted by the city nor municipal improvements made thereon until those streets have been named. If they are extensions of existing streets, the existing names shall be continued. If the new streets are not extensions of existing streets, the names recorded shall not duplicate or closely approximate street names already assigned.

EXCAVATIONS AND CONSTRUCTION
§ 95.10 OPENING PERMIT REQUIRED.
   It shall be unlawful for any person, other than the City Manager or other authorized city official, to make any opening in any street, alley, sidewalk, or public way of the city unless a permit to make the opening has been obtained prior to commencement of the work.
