§ 51.041 PERMITS; APPLICATIONS.
   (A)   Every application for a water or sewer connection shall state the following:
      (1)   The name of the owner of the lot;
      (2)   The name of the street on which the lot is situated;
      (3)   The number of the building if there is one on the lot or, if not, a description of the location of the lot;
      (4)   The number and kind of connections desired;
      (5)   The character of the surface of the abutting street; and
      (6)   The type of waste to be discharged into the sewer system.
   (B)   Every application shall be signed by the person making the application, shall be accompanied by the proper fee for making the connection applied for, and shall be filed with the City Water Department.
