§ 118.41 APPLICATIONS.
   Applicants for permits under this subchapter must file with the Police Department an application in writing on a form obtained from the Department which shall contain the following information.
   (A)   Name of the applicant.
   (B)   Address (legal and local).
   (C)   If the applicant is an employee or an agent of a company, the name and address of the employer.
   (D)   A statement as to whether or not the applicant has been convicted of any crime, misdemeanor or violation of any municipal ordinance, the nature of the offense and the punishment or penalty assessed therefor.
   (E)   The fingerprints of the applicant.
   (F)   A photograph of the applicant.
   (G)   The applicant's driver's license number.
