The application for a permit shall be submitted in such form as the Public Works Department may prescribe from time to time, and, insofar as applicable, shall follow generally the requirements prescribed by the city ordinances for applications for building permits, in addition to any other information prescribed by the Public Works Department. Such application shall be accompanied by a map or plan of such proposed work.
