Applicants for a license under this chapter must file with the Clerk a sworn application in writing on a form to be furnished by the Clerk, which shall provide the following information:
(A) Name and description of applicant.
(B) Address (legal and local).
(C) A brief description of the nature of the business.
(D) If employed, the name and address of the employer.
(E) The approximate number of employees or agents so to be engaged.
(F) Such other information as may be required by the Clerk.
