Every person, firm, corporation, association or partnership desiring to operate, maintain, open or establish within the city a business consisting wholly or in part of the sale of food supplies and food products at retail, shall make application for a license therefor to the City Clerk of the city, on a form prescribed and furnished by the City Clerk, which application shall set forth the name of the owner, manager, trustee lessee, receiver or other person desiring the license, the name of the store, the location, including the street number of the store, and the other facts as the City Clerk may require. If the applicant desires to operate, maintain, open or establish more than one place of business, he or she shall make a separate application for a license to operate, maintain, open or establish each place of business, but the respective places for which the applicant desires to secure licenses may all be listed on one application blank.
