(A) The application for a merchandise, service or music vending machine license shall contain the following information:
(1) Name and address of the applicant, age, date and place of birth;
(2) Prior criminal convictions of applicant, if any;
(3) Place where machine or device is to be displayed or operated and the business conducted at that place; and
(4) Description of machine to be covered by the license, mechanical features, name of manufacturer, serial number.
(B) No license shall be issued to any applicant unless he or she shall be over 18 years of age and a citizen or permanent resident of the United States.
(C) Application for a merchandise service or music vending machine license shall be made out in duplicate, one copy being referred to the Chief of Police and the other copy to the Director of the Department of Public Works.
(1) The Chief of Police shall ascertain if the vending machine applicant is a person of good moral character, by an investigation to determine if he or she was guilty of any criminal offense and based on the investigation either approve or disapprove the application.
(2) The Director of the Department of Public Works shall inspect his or her files to see if the commercial establishment wherein the licensed machine is located complies with the municipal ordinances of the city, and has a valid certificate of occupancy, and based upon this investigation either approve or disapprove the application.
