(A) An application for a license shall be made in writing and filed with the City Clerk.
(B) The application shall give the age and full name the applicant; the names and addresses of all persons interested in the issuing of the license; if the applicant be a corporation or the names and addresses of the officers; if the applicant be a partnership, the names and addresses of the partners and in addition stating where the person intends to do business, the kind of business and the length of time for which a license is desired.
