§ 121.71 LICENSE REQUIRED; APPLICATION; FEES.
   (A)   Any individual deserving to practice the profession of massage therapy anywhere within the municipal boundaries of the city is required to obtain a license from the City Controller before so practicing.
   (B)   There shall be an application filed containing the following information:
      (1)   Name and address of applicant;
      (2)   Social security number;
      (3)   Birth certificate showing applicant is at least 18 years of age;
      (4)   Proof of completion of 500 hours of schooling at accredited school or verification of testing results;
      (5)   Proof of accreditation of school;
      (6)   Prior employment history;
      (7)   Education history;
      (8)   Written blood test results evidencing that the applicant is free of any contagious or communicable diseases, including but not limited to: HIV, hepatitis and tuberculosis; and
      (9)   History of criminal conviction.
   (C)   There shall be a fee of $100 per license.
   (D)   All applications for license must be reviewed by a committee consisting of the City Controller, the Health Department Administrator and the Chief of Police or designee. License shall be issued only to those applicants who are approved by all three members of the review committee.
   (E)   License shall be good for a period not to exceed 1 year and regardless of when the initial license was issued, license must be reviewed by January 31 of each succeeding year.
   (F)   A late fee of $300 per license shall be assessed for any renewal applied after January 31.
