§ 112.066 APPLICATION; FEE.
   (A)   Any person desiring a license to operate, maintain or place any coin-operated record playing machine in any public place, as described in § 112.065, shall make application in writing therefor to the City Controller, setting forth in the application the following information.
      (1)   The full name and address of the owner of the machine;
      (2)   The full name and residence of the applicant;
      (3)   The address where the machine shall be operated, maintained or placed;
      (4)   The date the machine was purchased; and
      (5)   The nature of any encumbrance or lien upon the title to the machine and the name and address of the person holding title.
   (B)   Upon application for a jukebox license being filed with the controller, it shall be the duty of the Controller to present the application to the Board of Public Works and Safety for its approval or disapproval, as the case may be. The action of the Board of Public Works and Safety shall be at it its discretion and final, except as otherwise provided by law.
   (C)   All applications made under this subchapter for jukebox licenses shall be verified under oath and a fee of $10 per unit shall be included.
