(A) The Health Department of the city shall collect service fees as follows:
(1) Birth certificate: $12 each.
(2) Death certificate: $12 each.
(3) Searches (including genealogy): $25 each.
(4) Affidavit of amendments: $25 each.
(5) Court orders: $25 each.
(6) Paternity affidavits: $25 each.
(7) Notifications, birth to 2 years: $25 each.
(B) All fees collected shall be accounted for and transferred to the Health Fund of the city’s Health Department.
