(A) The Board of Public Works and Safety shall provide in triplicate serially numbered forms for notifying violators to appear and answer charges of violating the traffic ordinances of the city or of the state traffic laws.
(B) These forms shall be issued to and receipted for by the commanding officer of the Traffic Bureau or other persons acting for him or her according to serial numbers appearing on the forms.
(C) The commanding officer of the Traffic Bureau shall, when necessary, distribute the forms for notice of arrest to all police officers of the Department who shall sign receipts therefor according to serial number. The commanding officer of the Traffic Bureau or other persons acting for him or her shall receive from and receipt for all copies of the notices delivered by officers of the Police Department, as provided by this subchapter, and shall keep a record showing by serial number the notices issued and the disposal of the notices, whether the person charged with violations is fined, held in custody or released.
