§ 110.36 REPORT ON ALARM RESPONSE; REVIEW.
   (A)   A police officer responding to a police alarm shall prepare a report on his or her action and the actions of other responding officers regarding each alarm, including the determination as to whether it was a false alarm, what the weather conditions were at the time of the alarm, and the name of the individual with whom the officers conferred. The Police Department Alarm Citation form shall be used for this purpose.
   (B)   In the case of a fire alarm, the firefighter shall prepare a “run report,” on which he or she shall indicate whether the alarm was a false alarm. If this was a false alarm, the Run Report shall be forwarded to the Inspection Bureau of the Fire Department.
