Subdivision re-recording may be submitted at the convenience of the subdivider. The name of the subdivision shall not be altered. The submission shall include the following:
(A) The original drawing and five copies of the recorded subdivision with a revision date drafted onto the original plat;
(B) A written comment in detailed form of the technical changes performed;
(C) The seal and certification of the surveyor preparing the drawing changes;
(D) Proof of ownership or a copy of a purchase agreement; and
(E) The applicable fee as prescribed by the Commission.
