(A) Application for registration as heating, ventilating, and cooling contractor, hereafter referred to as a “heating contractor”, shall be made to the City Controller on a form furnished by him or her for such purpose.
(B) The form shall require the following information:
(1) Name, age, business and home address of applicant;
(2) A list of past associations with any business, firm, partnerships or corporation engaging in heating, ventilating and cooling work within the State of Indiana and/or Illinois;
(3) The name or names of any firm, business, partnership or corporation by who applicant is currently employed or for whom he or she is an officer or in which he or she owns any interest;
(4) The name or names and home addresses of each owner and officer of each business, firm, partnership or corporation referred to in division (B)(3) above; and
(5) If the applicant is to have his or her license used on behalf of a corporation, the name of such corporation. A heating contractor shall in all instances be a natural person of the age of 18 or more.
