The notice required in § 150A.04 shall be on a form approved by the town Building Official, shall include the words "Notice of Intent to Demolish" in prominent letters at the head of the notice and shall include the following information:
(A) The information required in § 150A.03;
(B) A statement that an application for a permit to demolish a structure has been filed in the office of the town Building Official and is currently pending and available for public inspection; and
(C) A statement setting forth the provisions of § 150A.06.
