(A) Each bed and breakfast shall apply for a license to operate within the city limits.
(B) This application shall include, but not be limited to:
(1) Information requested on a signed application form available at the municipal building;
(2) Signed affidavit stating:
(a) The operator will comply with all requirements of 50 ILCS 820/1-8;
(b) The operator has not been convicted of a crime in the last five years;
(c) The building has been inspected for fire hazards and meets State Fire Marshal’s requirements for one and two-family dwellings;
(d) The operator has completed State Food Service Sanitation course;
(e) Linens shall be clean and sanitary;
(f) Liability insurance is in force;
(g) All State Department of Revenue requirements have been met;
(h) The floor plan of bed and breakfast has been delivered to the City Fire Department; and
(i) Interior systems meet current city code requirements.
(3) Submit an application fee as set out in § 38.01 of this code of ordinances.
