§ 91.29 VILLAGE IDENTIFICATION TAG; DOG TAX.
   (A)   In addition to the inoculation tag required by § 91.25 of this chapter, every calendar year each owner of each dog over four months of age shall obtain from the Village Hall a serially numbered identification tag and pay a dog tax of $3 per dog which is hereby imposed.
   (B)   The Village Clerk shall issue the tag in the form prescribed by the President and Board of Trustees to an owner only upon receipt of:
      (1)   A signed application in the form prescribed by the Clerk containing the name, address, and telephone number of the owner and the name, if any, of the dog;
      (2)   A valid certificate of rabies inoculation showing an inoculation effective for the entire calendar year; and
      (3)   A dog tax of $3.
   (C)   The Clerk shall maintain a list of all outstanding dog tags, which shall include the name, address and phone number of each owner corresponding to each tag and the date of issuance of each tag. The Clerk shall provide the Chief of Police with a copy of the list.
   (D)   In years subsequent to issuance of the initial tag for any dog at the request of the owner, the Clerk shall renew the tag rather than issue a new tag, upon receipt of the documentation set forth in division (B) above and the dog tax of $3.
   (E)   The owner or keeper of each dog shall cause the village identification tag to be attached to a collar or harness worn by the dog at all times.
   (F)   Should a village identification tag be lost during a year of which the dog tax has been paid, the Clerk shall reissue the tag for the remainder of the year upon payment of $0.50.
