§ 155.027 APPLICATION PROCEDURE FOR SUBDIVISIONS.
   (A)   Application requirements.
      (1)   After the advisory meeting with the Administrator, and in order to begin the subdivision process, the applicant shall file with the Administrator an application for review of a sketch plan, as described in § 155.150 of this chapter, and all applicable fees.
      (2)   The application shall:
         (a)   Be submitted on forms available at the office of the Commission and be signed by the owner;
         (b)   Be accompanied by a fee of $50, plus $3 per lot in excess of four lots;
         (c)   Identify all contiguous property of the owner with an indication of the portion that is proposed to be subdivided, accompanied by an affidavit of ownership which shall included the date that each separate parcel of land was acquired, together with the book and page of each conveyance to the present owner as recorded in the office of the county’s Recorder;
         (d)   Be submitted in duplicate;
         (e)   Be accompanied by a minimum of three copies of the sketch plan; and
         (f)   Include the address and telephone number of an agent within the territory of the Commission who shall be authorized to receive all notices required by this chapter.
   (B)   Checkpoint submission. A copy of the sketch plan shall be submitted to each of the checkpoint agencies which have jurisdiction or authority over any area covered by the proposed subdivision, as listed below. The sketch plan and application will not be deemed to be submitted until all of the checkpoint agencies are in receipt of the sketch plan. The Administrator shall request that all checkpoint agencies and officials to whom a request is made submit a written report to the Administrator within 15 days of their receipt of the sketch plan. Such checkpoint agencies shall include the Town Council, Town Marshal, Zoning Commission, Utility Superintendents, Fire Department, the county’s Health Department and the Eastern School Corporation.
   (C)   Administrator’s review. Within 30 calendar days of the submission date, the Administrator shall study the proposal, review the application, review any checkpoint agency comments and meet with the applicant and any representatives of the reviewing agencies to discuss the aspects of the proposed subdivision and possible modifications or changes which may be required under this chapter. At this time, the Administrator shall also classify the proposed subdivision as either a major or a minor subdivision, as defined in this chapter.
