9-2-2-2: PRELIMINARY PLAT AND SUPPORTING DOCUMENTS:
The preliminary plat shall be drawn or printed on paper at a scale of one hundred feet to one inch (100' = 1"), unless a different scale is approved at the sketch plan conference. This plat shall show or be accompanied by the following information:
   A.   Title and Certificate: Name under which the proposed subdivision is to be recorded, location and position by quarter-quarter section, section, township, range, meridian, county, and state; names and addresses of subdividers; notation stating scale, north arrow; and the following certificate:

Name of subdivision must be original and not duplicate or be substantially similar to any existing Lake County subdivision.
   B.   Boundary Lines: Complete survey data such as angles, bearings, dimensions, curve data, etc., shall be shown for the exterior lines of the subdivision. The error of linear closure of said boundary survey shall be not greater than one in ten thousand (1 in 10,000).
   C.   Existing Easements: Location, width, and purpose of easements and other existing restrictions, reservations, or covenants.
   D.   Existing Streets: Existing streets showing pavement width, right-of-way width, and name; also existing curbs, gutters, culverts, sidewalks, and building setbacks.
   E.   Existing Utilities: Existing utilities showing location and size of sanitary and storm sewers; fire hydrants, and location and size of water mains; location of gas lines, electric and telephone lines.
   F.   Ground Elevations: Ground elevations on the tract, based on 1929 USGS Sea Level Datum, including: contours at two foot (2') intervals for land that slopes more than two percent (2%); contours at one foot (1') intervals for land that slopes less than two percent (2%); and spot elevations at all breaks in grade, along all drainage channels or swales, and at selected points not more than one hundred feet (100') apart in all directions. Contours shall not be taken from USGS maps.
   G.   Surface Conditions: Surface conditions on the tract including watercourses and surface drainage patterns, marshes, bogs, wooded areas, isolated preservable trees, natural prairie, and rock outcroppings. The limits and elevations of flooding as shown on the National Flood Insurance Program Flood Insurance Rate Map (FIRM) and Floodway Maps dated March 2, 1981, and amendments thereto shall be indicated.
   H.   Man-Made Features: Man-made features including houses, barns and other structures, and other constructed features.
   I.   Location Map: A small-scale two thousand feet to one inch (2,000' = 1") drawing of the region in which the subdivision is situated, showing the location of the subdivision, and indicating its relationship to traffic arteries, community facilities, railroads and other nonresidential land uses or adverse influences within a radius of two (2) miles. Current boundary lines of all municipalities within the area of this drawing shall be shown.
   J.   Proposed Public Improvements: Proposed public improvements for future construction on or near the tract, according to the information received from the Plat Officer at the sketch plan conference.
   K.   Subsurface Conditions: Subsurface conditions describing depth of ground water unless test pits are dry at a depth of eight feet (8'); location and results of any tests made to ascertain subsurface soil and rock conditions.
   L.   On-Site (Septic) Sewage Disposal Systems: A report from the Lake County Health Department indicating approval of the subdivision for septic systems shall be submitted for any subdivision with lots to be served by individual on-site (septic) sewage disposal. The report shall be prepared in accordance with the regulations and procedures of the Lake County Board of Health Ordinance, article V, rule II, which regulates the approval of new subdivisions throughout Lake County. In the event the said Lake County Health Department Regulations are amended or revised, the report shall be prepared in accordance with the then current regulations and procedures of the Department, relating to approval of the subdivision for septic systems.
   M.   Proposed Streets: Proposed streets showing right of way, pavement or roadway width and proposed names.
   N.   Proposed Easements: Proposed easements showing location, width and purpose.
   O.   Lot Lines, Building Setback Lines: Lot line approximate dimensions and building setback lines.
   P.   Reservations, Dedications: Sites to be reserved or dedicated for any public or semi-public use.
   Q.   Conservancy Areas: The conservancy areas shall be depicted on the face of the plat and the following language shall be included verbatim on the face of the plat:
Prohibited Uses:
1. No man-made structure of any kind shall be constructed in the flood plain.
2. The flood plain shall not be filled nor shall the grade be altered in any respect.
3. No materials shall be utilized or stored which shall have the potential for polluting either surface or ground water.
4. There shall be no floodway alteration.
5. There shall be no disturbing of natural vegetation.
   R.   Water Facilities: The location of all lakes, ponds, detention sites, retention sites and dams shall be depicted, and accurately located on the final plat.
   S.   Data Tabulation: Tabulation of data on gross area, street area, other dedicated area, net subdivided area, number of lots, average lot size, typical lot dimensions, and lineal feet of street.
   T.   Proposed Contours: Proposed contours of the finished subdivision.
   U.   Proposed Agreements: Proposed agreements, bylaws, provisions or covenants which govern the use, maintenance, insurance and continued protection of the proposed development and its common open areas and other facilities.
   V.   Supplementary Plans and Drawings: When unusual site conditions exist, the Plat Officer may require such additional plans, specifications and drawings as he finds necessary for an adequate review of the plat. The Plat Officer shall request this additional material from the subdivider by letter, stating therein the information requested and the particular or unusual site conditions which make this information necessary.
All of the requirements of plat preparation shall apply equally to the site being subdivided and to all land within two hundred feet (200') of its boundaries, waiving accuracy requirements as to scale of features but not as to boundary lines. (Ord. 90-03, 7-25-90)
