All permit applications shall be accompanied by three (3) sets of the following documents: a drawing of the site, with the location of all proposed fixtures depicted thereon. Concrete foundation detail indicating anchor bolts, conduits, ground rod and depth of foundation in relation to finish grade as required for specific soil conditions. Trench and compacted backfill material with size and type of direct burial cable. No open cutting of paved roadway is allowed. Light poles shall not be located closer than fifteen feet (15') to any fire hydrant, manhole, catch basin or inlet. Also required shall be statements of use intended, the power source, the type of light source, a representation of the actual fixture (original catalog cut and/or drawing), fixture height, material and finishes, photo cell location, source brightness in foot-candles and sufficient information to determine the amount of illumination which would shine on adjacent property. In addition, all other information and data reasonably necessary to evaluate the permit request, shall be provided to the Village Board as it requests. (Ord. 96-15, 7-31-1996)
