(a) Every applicant for a license as a driver of a taxicab shall make an application to the Mayor on forms to be supplied by the Mayor. The application shall set forth that
the applicant is:
(1) Able to speak, read and write the English language;
(2) Eighteen years of age or over;
(3) Free of defective vision, defective hearing, epilepsy, vertigo, heart trouble and any other infirmity, physical or mental, which would render him unfit for safe operation of a public vehicle; and
(4) Not addicted to the use of alcohol or drugs, and such other pertinent information as the Mayor may require.
The application shall be sworn to by the applicant.
The application form shall further contain the certificate of a reputable physician that the applicant is free from infirmities which would prevent him from driving a public conveyance with safety. (Ord. 72-9L. Passed 3-7-72.)
(b) All applicants for taxicab drivers' licenses shall present themselves at the Police station to be photographed and fingerprinted by the Police Department. After the Police Department furnishes the applicant with the photographs, one must be attached to the taxicab driver's license, which license shall be displayed in the taxicab while the driver is on duty.
