§ 156.17 APPLICATIONS.
    Application for a sign permit shall be made to the Code Enforcement Officer upon a form prescribed by the Code Enforcement Officer and shall contain all information necessary to enable the Code Enforcement Officer to determine whether the sign conforms with this chapter, including the name, address, and telephone number of the owner and/or lessee of the sign and of the land upon which the sign and sign structure is to be located, and a site plan which shall contain the following:
   (A)   Scale and North arrow;
   (B)   Address of site;
   (C)   All relevant property lines;
   (D)   Proposed ingress and egress to the site showing traffic flow;
   (E)   Location of all building and other structures on the site;
   (F)   Location and height of the sign;
   (G)   Dimensions of the sign;
   (H)   A drawing of the sign; and
   (I)   Any other information that the Code Enforcement Officer may reasonably require.
