§ 150.16 PERMIT APPLICATION; BOND; INSPECTIONS; CONDITIONS.
   (A)   Application for a permit required by § 150.15 shall be made by the property owner or his or her authorized agent, to the Director of Public Service or his or her appointee as enforcement officer, on forms provided by the city.
   (B)   A $40 nonrefundable permit fee, to cover the cost of application review and processing and inspection, shall be paid at the time an application for a permit is submitted. A $50 nonrefundable permit fee, to cover the cost of application review and processing and inspection, shall be paid at the time an application for a special hauling permit is submitted. Each trip one-way shall require a separate permit.
   (C)   A bond, if required, shall be determined by the Director of Public Service or his or her appointee, based upon the scope and extent of excavation required for each individual project. Such bond must be filed with the city prior to issuance of any permit. The bond shall be in the form of a commercial bond, certified check, cashier’s check or money order made payable to the city. Cash will not be accepted. Bonds required in the amount of $1,000 or less must be in the form of a certified check, cashier’s check or money order. The bond shall be in force and shall be released upon satisfactory completion of obligations set forth, as determined by the Director of Public Service or his or her appointee. The maximum bonding period shall be one year. Utility companies routinely working in the city may post one bond per year in an amount established by the Director of Public Service or his or her appointee.
   (D)   The first hour of inspection shall be included in the permit fee. Additional inspection time shall be billed to and paid by the permit holder. All inspection fees must be paid before release of the bond.
   (E)   The Director of Public Service or his or her appointee may impose such reasonable conditions or requirements upon issuance of a permit as he or she deems necessary or proper to assure compliance with city regulations and specifications.
