At least annually, in conjunction with regular sewer service charge billings, each user shall be notified of the rates and portions of such charges attributable to:
(a) Operation, maintenance and replacement (Sewer Revenue Fund).
(b) Debt service, etc. (Capital Improvement Fund).
(c) Surcharges, special assessments and other charges as applicable.
(d) Five hundred dollars ($500.00) fee per annum for Industrial Users.
