1244.07  FINAL PLATS; PREPARATION. 
   (a)   Final plats shall be prepared and certified by a registered surveyor in ink on tracing cloth or reproducible mylar at a scale of one inch equals 100 feet.  Sheet size shall be twenty-four inches by thirty inches.  Where it is necessary to use several sheets, said sheets shall be consecutively numbered and an index shall be provided showing the entire subdivision.
   (b)   Final plats shall include the following:
      (1)   The name of the subdivision, township, tract and original lot or section number.
      (2)   A north arrow, scale and date.
      (3)   A vicinity map at a scale of one inch equals 1,000 feet.
      (4)   The accurate location of all survey monuments found, set and required to be set.
      (5)   A summary of site data, including the total number of lots, the average lot size, the minimum lot size and the amount of acreage in lots, rights-of-way, parks, open space and other public uses.
      (6)   Names of recorded owners of adjoining, unplatted land and reference to subdivision plats of adjoining platted land by name, volume and page of the recorder's map.
      (7)   Property boundaries, centerlines and right-of-way lines of streets, easements and other rights-of-way, natural and artificial watercourses, streams, shorelines, corporation lines and property lines of all lots and parcels with distances, radii, arcs, chords and tangents of all curves shown to the nearest one hundredth of a foot, and bearings or deflection angles shown to the nearest second.
      (8)   Building setback lines accurately shown with dimensions.
      (9)   A certification and seal by a registered surveyor to the effect that the plat represents a survey made by him or her which balances and closes, that the monuments shown thereon exist or shall be set as shown and that all dimensions and bearings are correct.
      (10)   A notarized certification by the owner that the plat was prepared with his or her consent and offering for dedication the streets, easements and other public areas.
      (11)   A release signed by the mortgage holder, if any, of all interest in properties dedicated to public use.
      (12)   An approval block for the endorsement of the plat by the City Manager.
      (13)   Proper notations for transfer and recording by the County Auditor and the County Recorder.
      (14)   A copy of proposed covenants and restrictions, including perpetual maintenance provisions for proposed common areas.
      (15)   A proposed entrance, landscaping and street tree planting plan.
      (16)   The location and design of required buffers.
   (c)   All dimensions, angles and bearings shall be referred to control points, the  nearest established street line, section lines or other established points.
   (d)   Lots shall be numbered in consecutive order and, when the subdivision is submitted in sections or phases, lots shall be numbered consecutively as each section or phase is submitted.  In cases where lots or parcels have previously been transferred, such lots or parcels previously transferred should also be numbered in sequence with all new lots created in the proposed subdivision.
   (e)   Final plats shall be accompanied by detailed construction plans prepared and certified by a registered professional engineer for all required improvements.  Said plans shall be at a minimum scale of one inch equals fifty feet on twenty-four-inch by thirty-six-inch sheets.  Said plans shall include profiles, elevations, construction details, specifications and calculations and computations for all required improvements.  Improvement plans shall also include an overall grading and stormwater management plan and an erosion control plan.
