Applications for vehicle or helicopter permits shall be made on forms prepared and approved by the Medical Director and contain such information as required by the Medical Director. Each application shall be accompanied by a non-refundable application fee in the amount of $50 payable to the EMS Foundation, except that no charge shall be made for vehicles operated by volunteer providers.
