The permit required by § 99.94 shall be based upon a formal petition in writing, addressed to the Board of Public Works. The petition shall distinctly set forth the points from and to which such removal is to be made, shall accurately describe the building and shall set forth specifically the route to be taken and the streets to be passed over.
