§ 6-5-13 APPLICATION.
   All new water customers moving into a dwelling must complete an application form for new water user’s account. All forms provided by the city shall be signed by the customer for the use of water service. All applications will be located at City Hall, 811 Avenue E, Fort Madison, Iowa.
