§ 51.090  PERMIT APPLICATION; FEE.
   (A)   Significant Industrial User Permit Applications.
      (1)   Significant industrial user permit applications shall make application for the proposed discharge on a form provided by the Utility Service Board. The permit application shall be supplemented by any plans, specifications, studies, or other information considered pertinent by the Superintendent, to evaluate compatibility with the sewerage system.  It is the purpose of charges and fees to provide for the recovery of costs from Users of the POTW for the implementation of the program established herein.  The applicable charges or fees shall be set forth in the Board's schedule of charges and fees.
      (2)   The Board may adopt charges and fees which may include those set out in divisions (a) through (g) below.  These fees relate solely to the matters covered by this chapter and are separate from all other fees chargeable by the Board.
         (a)   Fees for reimbursement of costs of setting up and operating the POTW's pretreatment program.
         (b)   Fees for monitoring, inspections, and surveillance procedures.
         (c)   Fees for reviewing accidental discharge procedures and construction.
         (d)   Fees for permit applications.
         (e)   Fees for filing appeals.
         (f)   Fees for consistent removal (by the POTW) of pollutants otherwise subject to federal pretreatment standards.
         (g)   Other fees as the Board may deem necessary to carry out the requirements contained herein. 
      (3)   Permit fees for major contributors shall be as follows.

   (B)   Minor contributions.  A fee of $25 will be paid for each minor contributor permit.
