(A) At least 35 days prior to the scheduled date of the event, any not-for-profit charitable organization proposing to conduct an event under the Charitable Games Act, ILCS Ch. 230, Act 30, §§ 1 et seq., shall file with the Village of Frankfort Police Department a fully completed copy of its State of Illinois Department of Revenue Application to conduct charitable games file-stamped by the State of Illinois and as necessary supplemented to include the following information and submittals:
(1) The name, address, phone number, email address of the charitable organization conducting the charitable game event;
(2) The name, address, home, business and cell phone numbers and email address of the contact person for the charitable organization who is principally responsible for the event;
(3) A copy of the application filed by the charitable organization with the State of Illinois under ILCA Ch. 230, Act 30, §§ 1 et seq.;
(4) A copy of the license issued by the State of Illinois authorizing a charitable games event for the planned date of the event;
(5) The date of the event;
(6) The name, address and phone number of the venue where the event will be held, the time the event will commence and conclude;
(7) Information concerning event security;
(8) Information concerning traffic control and vehicular parking; and
(9) Acknowledgment that the charitable organization will fully comply with all applicable requirements under ILCS Ch. 230, Act 30, §§ 1 et seq., the Village of Frankfort liquor control ordinances and all other applicable laws and ordinances.
(B) These submittals will constitute the charitable organization's Village of Frankfort event registration and license application.
